$19.96 - $27.94The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
GENERAL PURPOSE:Manages, monitors, troubleshoots, analyzes, and evaluates the viability and integrity of patient data housed in all Health Information Management Systems (HIMS) in support of patient care and utilized in the storage, retrieval, data abstraction, reporting, and accessibility of the patient medical records. Reviews, researches, and resolves problems and reduces errors concerning the Master Patient Index (MPI), patient identity, duplicate medical record numbers (MRNs), and merging of patient records. Monitors work queue, requests and/or problem tickets and interacts with other departments and resources to research documentation discrepancies. Provides resolution of patient linking or duplicate medical record numbers that could lead to patient safety issues. Facilitates the chart correction process with end users by following regulatory requirements and set forth policies and procedures to coordinate timely corrections and perform resolution verification.
ESSENTIAL FUNCTIONS
- Controls the accuracy of all static data changes within the EMR and validates and maintains ongoing data integrity. Ensures chart correction is performed accurately and corrections are reflected in all systems.
- Identifies possible duplicate medical record numbers, overlays, and data integrity issues through the use of daily reports and work queues.
- Maintains patient identity integrity by using analytical and investigational skills and multiple tools and systems to piece together the patient identity story and independently determine, coordinate, and execute action needed to remediate identity discrepancies.
- Serves as a liaison between HIM, Patient Access, clinical areas, and information systems related to all EMPI processes, including patient integrity. Performs system functions as needed for building and testing system changes and system go lives.
- Assesses end-user training needs and provides needed education and support.
- Coordinates EMR downtime with appropriate IT and HIM Leadership.
- Reviews and identifies deficiencies, discrepancies or omissions of information from the various EHR’s and communicates to medical staff, nursing staff or appropriate system contact or responsible individual. Reviews medical records within the EHR to ensure completeness in accordance with hospital policy and all needed documentation is available for coding and billing processes. Adds deficiencies to electronic health record if they have not been auto created. Works with providers to complete required documentation prior to provider suspension.
- Participates in a rotational on call coverage to provide Data Integrity support to clinical areas 24x7/365 as needed for patient care needs. Analyzes and determines appropriate course of action, involving other teams as needed to ensure timely and accurate resolution.

MINIMUM EDUCATION & EXPERIENCE
- Associate's degree and 2 years of related work experience in a clinical setting required
- Or equivalent combination of education and experience
KNOWLEDGE, SKILLS AND ABILITIES
- Ability and temperament to work with minimal to no supervision and to exercise independent judgement in problem solving with organizing multiple tasks/projects, and setting priorities effectively required
- Exceptional PC skills, software skills, ability to function well in a team environment, effective communication, analytical and problem-solving skills required
- Familiarity with hospital registration and billing systems, EMRs, insurance and provider databases preferred
