A division of the Primary and Community-Based Care portfolio, the Patient Access and Navigation Services (PANS) team is focused on enabling access to care through Health811 and key domain areas such as mental health & addictions, primary care, virtual urgent care, and the adoption and implementation of online appointment booking solutions.

The Lead, , Patient Access and Navigation Analytics and Innovation will be responsible for leading the Health811 data, reporting, analytics and innovation roadmap, with a focus on understanding patient flow across the health system. Using administrative health data this role will perform continuum of care and outcome analyses to assess system impact and effectiveness. Additionally, this role will play a key part in shaping Health811’s AI strategy, leveraging artificial intelligence and predictive analytics to enhance reporting, service delivery insights, and system performance monitoring. The Lead will explore opportunities to apply AI and machine learning to Health811 data, helping to refine patient navigation, optimize service utilization, and proactively identify system trends. The Lead will work closely with vendors and internal Ontario Health teams to monitor and evaluate service performance, reviewing vendor reports and providing critical analysis to OH leadership to inform key decision-making. They will lead the development and implementation of reporting indicators, data collection processes, and analytic mechanisms that support PANS program operations, performance measurement, and evaluation needs. Additionally, the Lead will collaborate across Ontario Health to drive data quality, program evaluation, system performance monitoring, and continuous improvement efforts. This role will also identify opportunities for transformational change founded in value-based care and support the implementation of Value-Based Incentive frameworks for Ontario Health and system investments, including Health811.

Here is what you will be doing:

As the Lead, Patient Access and Navigation Analytics and Innovation you will play a critical role in shaping the data analytics and innovation operating model for Health811 and the Patient Access and Navigation Services (PANS) team. Your work will directly support system performance, decision-making, and value-based care initiatives. Key responsibilities include:

  • Subject Matter Expertise (SME): Serve as a content and technical expert on Health811 and PANS-related reporting, data management, data quality, and analytical methodologies. Contribute expertise to system- and regional-level performance measurement indicators.

  • Data Strategy & Reporting: Design, develop, and manage analytical tools, dashboards, surveys, and templates to support Health811 and PANS program and project reporting. Lead corporate reporting efforts, including scorecards and key performance indicators.

  • AI Strategy & Implementation: Lead efforts to support the implementation of AI-driven enhancements aligned with Health811’s AI strategy. Identify opportunities where AI can improve data accuracy, patient navigation, predictive analytics, and overall service performance. Work closely with vendors and internal teams to explore AI solutions that optimize reporting, automate insights, and drive system improvements.

  • Data Quality & Vendor Oversight: Work closely with vendors to monitor and improve the quality of data received. AI knowledge and experience will be helpful to explore opportunities for enhancing data accuracy, validation processes, and reporting improvements.

  • Accountability & Compliance: Ensure timely internal and external reporting for Ontario Health (OH) annual business plan reporting, Ministry of Health (MoH) reporting, and partner ad hoc reporting requests. Maintain compliance with data standards, approvals processes, and production requirements.

  • Data-Driven Storytelling: Translate complex data into meaningful, actionable insights that are easily understood by key stakeholders, supporting evidence-based decision-making.

  • Data Innovation: Identify novel datasets that can enhance population health and value-based care initiatives.

  • Critical Thinking & Strategic Insights: Analyze program and project data to identify trends, challenges, and opportunities, supporting strategy development and decision-making.

  • Collaboration & Stakeholder Engagement: Build strong relationships across Ontario Health to understand business needs, data priorities, and performance metrics. Partner with internal teams, advisors, and external system partners to enhance data reporting, visualization, and storytelling.

  • Partnership & Representation: Participate in advisory committees, working groups, and Ontario Health initiatives as a data and reporting SME, contributing to the advancement of Population Health and Value-Based Health Systems (PHVBHS) priorities.

  • AI-Enabled Continuous Improvement: Stay current with AI advancements in healthcare analytics and champion AI-enabled methodologies to enhance Health811’s data reporting, decision support, and predictive analytics capabilities.

  • Additional roles & responsibilities may evolve upon advancement and growth of the portfolio.

Here is what you will need to be successful:  

Education & Experience

  • Undergraduate degree in a relevant discipline (e.g., Data Science, Health Informatics, Business Analytics, or a related field). A master’s degree in a relevant discipline is preferred.

  • Minimum 5 years of experience in data, reporting, and analytics, preferably in a health-related field which includes hands-on experience with data visualization tools (SQL or similar) to create meaningful insights.

    • Experience with programming and statistical languages such as SAS and SQL is required. Proficiency in AI/ML tools like Python, TensorFlow, or R is considered an asset. At OH, SAS, R, and Python will be used to generate reports with aggregate data, while SQL will support data validation and analytical tasks.

    • Proven experience in leading AI and data-driven projects, defining requirements, and engaging partners to drive insights for decision-making.

    • Experience working with large data sources and linking large complex data sources

    • Experience working with Business Intelligence tools

    • Experience working with health administrative and clinical data sources (e.gNational Ambulatory Care Reporting System (NACRS), Electronic Canadian Triage and Acuity Scale (eCTAS), Ontario Health Insurance Plan (OHIP) Claims History Database (CHDB), Discharge Abstract Database (DAD), Registered Persons Database (RPDB), Client Agency Program Enrolment (CAPE), Ontario Health Teams Attribution Methodology (OHTAM), Ontario Marginalization Index (ON-Marg), and Reference Data (RefData) is an asset

  • Experience supporting and enabling large-scale strategic projects and/or innovative programs in complex clinical environments is preferred.

    • Proven experience in leading projects, defining requirements, and engaging stakeholders to drive data-driven decision-making.

    • Minimum 5 years' experience in collaborating with partners to gather, analyze, and report data. Skilled in engagement and consensus-building to ensure reporting and analytics initiatives align with business needs

  • Experience working with Microsoft products (Excel, PowerPoint, Word, Teams etc.)

  • Indirect leadership experience, including mentoring, guiding, and providing expertise to cross-functional teams to drive reporting and analytics initiatives.

Knowledge & Skills

  • Strong knowledge and understanding of AI applications in healthcare, including NLP, machine learning, and predictive analytics.

  • Ability to work with minimal stakeholder input to proactively develop impactful reporting and visualization tools (PowerBI, SQL or similar), while translating business and partner needs into actionable, data-driven insights and analytic products.

  • Strong database management, analytical, and problem-solving skills, ensuring data integrity and best practices in data handling.

  • Knowledge of health equity data, patient experience metrics, and outcome measurement for diverse populations is an asset.

  • Proficiency in qualitative and quantitative data methodologies for analysis and reporting.

  • Knowledge of Ontario’s health data landscape (e.g., patient access models, virtual care analytics, AI-powered health insights) to identify system improvement opportunities.

  • Excellent presentation and communication skills to convey insights and recommendations effectively.
