The Role
To ensure that the social Care Network (SCN) effectively tracks and manages client progress, one Data Manager will support program and administrative staff with accurate and timely data entry and data management in the UniteUs platform for the following service areas: Screening, Navigation, Transitional Housing, Outpatient Treatment, and Residential Treatment. Under the general direction of the SCN Supervisor/Coordinator, Assistant Director The SCN Data Manager will collect, analyze, monitor, and report internal and external EHRs (Avatar, UniteUs, FCM, DOH and Lead Health Homes) data regarding Hope Care Management Health Homes program. The SCN Data Manager will effectively combine diverse data sets and report the findings to the SCN Supervisor/Coordinator, Assistant Director, Director and Executive Leadership.
Responsibilities
What You Will Do
- Analyze data and processes to determine changes to existing reports or new reporting needs that will support program objectives and provide insight into the effectiveness of program initiatives.
- Conducts PC-MVS reports bi-weekly to distribute to Social Care Network Team.
- Collects, analyzes, and reports critical client data to improve service coordination and client outcomes.
- Maintains compliance with SCN data-sharing requirements and privacy regulations.
- Supports staff in utilizing data-driven insights to enhance service delivery and efficiency.
- Ensures real-time data entry and accuracy to track referrals, service engagement, and care transitions.
- Facilitates interdepartmental collaboration to address gaps in service and improve client experiences.
- Evaluates encountered data and identifies errors created by operational, claims, and eligibility processes.
- Collects and distributes utilization data to the SCN Supervisor and Assistant Director to address client needs.
- Coordinates with the Social Care Network and Managed Care Organizations to address ongoing segment issues and enhance program outcomes.
- Reviews and evaluates discrepancies within reports created by the Lead Health Homes and internal data; reports all issues to Program Director, promptly.
- Attends department, DOH, Lead Health Homes and Social Care Network meetings as needed/requested.
- Perform other duties as required.
Qualifications
Who You Will Be
- Bachelor’s in Accounting, Finance, Business Administration, or related field.
- At least Two-Three (2- 3) years’ experience in billing.
- Prior work experience in Health Homes or Care Coordination program.
- Computer literacy including proficiency in Microsoft Office Suite and EHR.
- Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
- Excellent interpersonal, organizational skills, analytical and problem-solving skills.
- Attention to detail and high level of accuracy.
- Knowledge of office administration and billing procedures.
- Ability to work independently with little supervision.
