Tasks

• Assist with obtaining signatures on forms
• Collect documentation for eligibility determination
• Conduct home visits to complete forms
• Coordinate with Department of Public and Social Services
• Educate participants on form completion
• Identify participants for Medi Cal redetermination
• Maintain compliance with DHCS and CMS policies
• Manage follow ups with participants and agencies
• Resolve eligibility issues with minimal direction
• Submit benefit applications
