- Assist with health care coverage enrollment and education
- Conduct outreach to potential patients in assigned areas
- Coordinate community outreach activities and in reach events
- Coordinate outreach and enrollment program
- Liaise with health plan representatives and insurance brokers
- Maintain outreach equipment inventory
- Monitor call logs and event tracker
- Provide patient navigation and support social determinants of health
- Track redetermination dates and follow up patients
- Train and mentor enrollment and retention specialists
