Tasks
- Assess transitional needs from inpatient to outpatient
- Assist with post discharge needs prescriptions transportation DME and appointments
- Conduct motivational interviewing
- Create referrals to appropriate agencies and resources
- Document accurately in electronic medical record
- Identify patients with chronic disease diagnoses
- Maintain competence in evidence based nursing practice
- Notify providers and RN of health condition changes
- Participate in daily huddles and patient care coordination meetings
- Perform clinical tasks within scope of practice
- Perform focused assessment of chronic care conditions
- Perform medication reconciliation
- Prepare reports for weekly meetings
- Provide education during discharge
- Provide patient education using approved materials
- Review contingency plan
- Support self management goal setting
