Care Coordinator, Embedded (Reston, VA - Woodburn)
Tasks
- Collaborate with providers within long term care setting
- Communicate patient needs changes in condition and urgent concerns to clinical team
- Coordinate care for long term care residents using EMR
- Document patient interactions and care coordination activities
- Follow workflows documentation standards and escalation protocols
- Maintain on site engagement with residents to support care plans
- Serve as liaison between providers facility staff residents and families
- Support team collaboration and facility partnerships
