Enrollment Coordinator
Tasks
- Collect treatment goals and determine eligibility
- Conduct phone intakes for health information
- Coordinate with care team for transition into treatment
- Maintain patient communication and rapport
- Outreach and engage via email SMS phone live chat
- Participate in enrollment process improvement
- Provide community resource referrals
- Provide treatment model overview
- Register patients and document in EHR
- Schedule first appointments
- Schedule intake calls
- Serve as First Point of Contact
- Update CRM to track enrollment pipeline
- Verify insurance coverage
