- Audit home health charts
- Check eligibility for state funded program disqualifiers
- Collaborate to review home care plan of care
- Complete DHCS level of care forms
- Conduct home visits
- Conduct pre enrollment assessment
- Determine home health and home care needs
- Develop home care plan of care
- Educate prospective participants on PACE enrollment requirements
- Evaluate PACE eligibility using criteria
- Perform level of care assessment
- Present candidates to interdisciplinary team
- Review outsourced home care and home health services weekly
