Tasks
- Collaborate with healthcare providers in long term care
- Communicate patient needs changes in condition and urgent concerns
- Coordinate long term care for residents using EMR
- Coordinate with facility staff and manage care liaison responsibilities
- Document patient interactions and care coordination activities
- Follow documentation standards and escalation protocols
- Maintain on site engagement with residents and families
- Support care plans follow ups and care transitions
