Care Coordinator, Embedded (Reston, VA - Springfield)
Tasks
- Collaborate with clinical providers in long term care
- Communicate patient needs condition changes and urgent concerns
- Coordinate care for long term care residents using EMR
- Coordinate with facility staff to support partnerships
- Document patient interactions and care coordination activities
- Follow workflows documentation standards and escalation protocols
- Maintain on site resident engagement
- Serve as liaison between providers facility staff residents and families
- Support care plans follow ups and care transitions
