Care Coordinator, Embedded (Atlanta, GA - Dunwoody)
Tasks
- Collaborate with providers in long term care setting
- Communicate patient needs changes and urgent concerns to clinical team
- Coordinate care for long-term care residents
- Document patient interactions and care coordination activities
- Follow workflows documentation standards and escalation protocols
- Maintain on-site engagement with residents
- Serve as liaison between providers facility staff residents and families
- Support care plans follow ups and care transitions
- Support team collaboration and facility partnerships
