Care Coordinator, Embedded (Manassas, VA)
Tasks
- Collaborate with healthcare providers
- Communicate patient needs and condition changes to clinical team
- Coordinate care for long term care residents using EMR
- Document patient interactions and care coordination activities
- Follow workflows documentation standards and escalation protocols
- Maintain on site resident engagement
- Serve as liaison between providers facility staff residents and families
- Support team collaboration and facility partnerships
