Enrollment Specialist/Care Navigator (Bilingual: English & Cantonese)
Tasks
- Answer patient inquiries and escalate concerns
- Communicate with patients caregivers and providers
- Coordinate patient care across programs
- Enter and maintain accurate EMR records
- Follow up with patients to support engagement
- Guide patients through enrollment and onboarding
- Monitor and manage referrals leads and opportunities
- Order and set up RPM devices
- Perform pre appointment chart cross checks
- Schedule and manage patient appointments
- Verify insurance eligibility
