Tasks
- Assign cases and coordinate client staffing
- Conduct oversight during consumer transition to the Waiver
- Conduct workstream meetings
- Ensure case closure compliance
- Ensure completion of iConnect steps and required forms
- Monitor iConnect steps before case closure
- Monitor work productivity and performance goals
- Participate in community events
- Plan and direct staff schedules
- Prepare monthly reports
- Provide guidance on non waiver service referrals
- Provide orientation and training to new staff
- Represent agency at transportation disadvantage meetings
- Review crisis enrollment requests
- Review service funding requests and invoices
- Supervise and evaluate staff work product
- Transfer client central records with required elements
